Your Visa® Gift Card is issued by Pathward®, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. See the expiration date below your account number for a valid thru date. If available funds remain on your Card after the valid thru date, call customer service to receive a replacement Card at no extra charge. Funds do not expire. Subject to applicable law, a Monthly Maintenance Fee of $4.95 will be deducted on the first day of the 13th month, following 12 months of inactivity. If your Visa Gift Card is lost or stolen, there is a $5.00 Replacement Fee to issue a new Card. This Visa Gift Card can be redeemed everywhere Visa debit cards are accepted in the United States, including online. This Card may not be used at any merchant outside of the United States. This Card is non reloadable. No cash or ATM access. The Visa Gift Card cannot be used to make pre-authorized or recurring bill payments. This Card is not refundable at retail locations.
